HR Forms and Resources

Below you will find links to various Human Resources forms that employees may need to complete during the course of their employment or affiliation with the Elk Grove Unified School District.  Completed forms along with supporting and/or required documents may be delivered to the Human Resources department Monday through Friday from 8:00 a.m. to 5:00 p.m.  For assistance you may contact the Human Resources department via email at egusdhr@egusd.net or by telephone at (916) 686-7795.

FORMS

Catastrophic Leave

Catastrophic Leave request may be submitted by Catastrophic Leave bank members. The purpose of Catastrophic Leave is to provide a way to help employees who have catastrophic events that require them to be away from work to recover or to care for an immediate family member after the employee has exhausted all other available types of leave.
 

Change of Name

Change of Name form is to be completed, signed, dated and submitted for processing.

 

Note: Name Change requires the original document be personally hand delivered to Human Resources with your original Social Security card for inspection and copying. Prompt updating with Human Resources is strongly recommended.

 
 

Change of Address/Phone Numbers

Employees have the ability to request an update to their address and/or phone number through the Employee Self Service (ESS) application.
 

COVID-19 - Paid Leave

The current COVID-19 leave is effective 10/1/2021 – 9/30/2022. This leave entitlement provides a maximum of 10 days of paid leave.

 


Emergency Leave

EGEA Emergency Differential Leave was a Pilot Agreement for 2017-2018, 2018-2019, 2019-2020 and was extended to include 2020-2021 and 2021-2022.  In the event a unit member exhausts all accumulated sick leave, unit members can use up to 2 days of Emergency Leave each school year during the term of the Pilot Agreement for those reasons identified on the verification form. All Emergency Verification Forms should be submitted directly to: leaves@egusd.net
 

Request for Leave of Absence

The below Request for Leave documents are to be completed by the employee, signed by their supervisor and returned to the Human Resources department along with stated supporting documentation.
 

Retirement and Resignation

Resignations and retirements may be submitted on the designated Resignation or Retirement forms by simply completing the applicable information, signing, dating and submitting the original document. The correct form for your classification and reason for separation, must be used so as not to delay processing.
 
Should you be planning to resign or retire, it is very important you refer to the Certificated or Classified Steps on each of the Resignation or Retirement documents. The document will provide you step by step instructions as you plan. Please ensure you have checked off each step prior to submitting your resignation or retirement date.

Employee Self Service application (ESS)

Elk Grove Unified School District offers its employees access to information on file with the District. Access personnel information, credentials, degrees, pay stubs/history (starting with the June 17, 2021 pay stub), and W2 information.
